Whenever you drink alcohol in Tibia, you will get drunk.
While being drunk you will have the drunk icon on your status bar.

As long as you are drunk, you are unable to completely control your movement (you will not walk the way you wanted to walk). Being drunk while fighting creatures can result in severe injuries, or even death.

Be aware, especially when hunting Djinns, Wyverns, Voodoo Cultists and Dworc Voodoomasters. They will induce heavy drunkenness upon a character. Wearing a Dwarven Ring is beneficial when hunting these, as it protects against the effects of drunkenness.
